
Community Care Program
Dean's Community Care Program is a health
benefits counseling program that helps uninsured or under-insured
patients determine their eligibility for government or community
funded financial resources. For patients not eligible for public
assistance, Community Care may also provide temporary and limited
assistance for Dean Health System physician related charges.
Community Care will assist patients in accessing public assistance programs that include:
- Medical Assistance
- Wisconsin
BadgerCare

- Wisconsin
Medicaid Presumptive Eligibility for Pregnant Women

- Wisconsin Medicaid Family Planning Waiver

- Wisconsin
Well Woman Program

Depending on each individual's situation and eligibility, Community Care will help patients access medical and family assistance resources that are available in Wisconsin.
Who is Eligible for Community Care?
All Dean Health System patients who are uninsured and meet the financial guidelines of the program are eligible to apply for Community Care.
Interested patients should complete a Dean Community Care application.
Applications can be obtained at any Dean Clinic or Dean/St. Mary's Regional Clinic reception desk, or by calling one of our community care specialists listed below.
