Form of ID - Please be prepared to provide a photo ID when you check-in for your appointment. We do not need a picture to provide care and will not withhold treatment due to lack of identification. Acceptable forms of ID include Driver's License, State Issued ID, Student ID, Military ID or Proof of Residence (Utility Bill, Bank Statement, Pay Check). Mary's Regional Clinic. It is part of one of the largest
regional physician practices in Wisconsin. A joint venture of
Dean Medical
Center and St. Mary's Hospital (both in Madison),
the network is comprised of more than 30 clinics. Portage Clinic
joined Dean Health System in 1997. A Dean
Specialty Clinic is conveniently located on the second floor
of the Portage Clinic building.
