Flu Season Alert
Get reimbursed up to $25 for your seasonal flu vaccination.
Now Dean Health Plan members can be reimbursed up to $25 for seasonal flu (not H1N1) shots received from any area provider, pharmacy or vaccination location.
- No charge when members get a shot from your Dean Primary Care Provider. Simply make an appointment at your convenience.
- Or get your seasonal flu shot at a location of your choices and submit this Seasonal Flu Shot Claim Form with a copy of your receipt to Dean Health Plan. Please submit one form per shot.
Note:Additional vaccination locations are for seasonal flu shots only. H1N1 vaccinations must be administered by a physician in a clinical setting. Please contact your clinic for an H1N1 vaccination appointment.
