Coverage of any intervention referenced in a Dean Health Plan/Premier Medical Insurance Group (DHP) medical policy is subject to the terms of the applicable policy, including limitations and exclusions outlined in the member's plan documents and is subject to state and federal laws. To the extent there are any conflicts between a DHP medical policy and the member's plan document language, the plan document language will govern.
These medical policies are for informational purposes only. An oral request for a referral does not guarantee authorization of the referral or the services. The member and requesting provider will receive written notification from the DHP Medical Affairs Division after review of an electronic or written referral request has been completed.
