
Fringe Benefits Summary
Earned Time Off
Earned Time Off is a program designed to combine traditional
paid vacation and sick time into one account, allowing employees
the ability to use this time at their discretion with supervisory
approval. Traditional holidays are not considered to be part
of the earned time program. Earned time applies to full time
employees and is prorated for part time employees.
Holiday Pay
Full and part time employees are eligible for holiday pay.
Short
Term Disability Insurance
Full and part time employees are eligible to participate
in a short term disability program which will provide income
replacement
when unable to work due to a certified medical disability.
Long
Term Disability Insurance
Dean pays the premium for long term disability insurance for
eligible employees. The benefit provides 60% of base income
(up to a monthly maximum) when medically unable to work.
Health
Insurance
Full and part time employees are eligible for health insurance
through Dean Health
Plan. Dean pays 80% of the premium for
full-time employees.
Dental Insurance
Full and part time employees are eligible for dental insurance
through Delta Dental. Dean pays 80% of the premium.
Life Insurance
Dean provides each full and part time employee with a term
life insurance benefit of one times annual salary up to $100,000.
Eligible employees may purchase additional life insurance
for
themselves, their spouse or dependent children.
Retirement
Plans
Dean offers a voluntary 401(k) plan with both pre-tax and post-tax
options. Following one year of service and 1,000 hours worked,
employees are eligible for a 3% fixed contribution and profit
sharing.
St. Marys Dean Ventures offers a voluntary 401(k) plan with both pre-tax and post-tax options. Following one year of service and 1,000 hours worked, employees are eligible for a 5% employer contribution. Advanced practitioners and providers are eligible for an 8% employer contribution.
Flexible Benefit Plan
Employees in a full or part time status are eligible to participate
in a voluntary flexible benefit plan and may set aside pre-tax
dollars for dependent day care and/or non-covered medical
expenses.
Tuition Reimbursement
Dean offers tuition reimbursement to encourage employees to
upgrade their skills in areas directly related to their present
position.
Full and part time status employees are eligible for reimbursement
of 75% of tuition costs up to a maximum of $2,500 per calendar
year.
Employee Assistance Program
The EAP is a free and confidential resource of information
and assistance to help employees and family members find
solutions
to personal problems that may affect health, family or job.
This assistance covers a broad range of concerns including:
family and relationship problems; emotional problems; physical
illness and stress; legal; financial and vocational issues;
and alcohol or other drug problems.
Pharmacy Discount
Employees are entitled to a 10% discount on non-prescription
medications purchased at Dean
Pharmacy locations and a 25%
discount on Leader brand over-the-counter products.
Vision/Optical
Discount
Employees and their dependents are eligible for discounts
on vision services and products. Receive a 40% discount on
complete
set of prescription glasses or sunglasses; discounts on non-prescription
sunglasses, contact lenses and refractive eye surgery.
Retiree
Health and Dental Insurance
Employees, whose age plus years of service equal 60 or more,
may continue their health and dental insurance under a retiree
program. The retiree pays the full cost of these benefits
and may continue the benefits for life.
